• Insurance card
• Name of your primary physician
• Name of referring physician
   

2. Schedule your appointment:

3. Confirm your appointment:

   We will call you prior to your appointment to confirm it. If we are unable to reach you we will leave a message. In this case, please call us back at 1 (949 824 0606).
    

4. Your Office Visit:

   For Medical or Cosmetic Appointments, don't forget to bring your:

   • Insurance card
   • Patient information and health history forms
   • Physician referral: Please bring your doctors referral form or letter
   • Appointments regarding cosmetic and aesthetic concerns cannot generally be billed to insurance and referrals are not necessary.

   For Aesthetic Appointments:
   
   Please arrive 10 minutes before your scheduled appointment to fill out any necessary paperwork.
